I understand that my pet’s comfort and safety are The Pet Doctor’s first priority. Every effort will be made to ensure that my pet’s grooming is pleasant and safe; however, if my pet becomes ill or hurt and the services of a veterinarian are required, I hereby give my permission for treatment to be given and agree to pay for such expenses. *

I give the Pet Doctor Staff permission to bathe and groom my animal. I realize that grooming requires the use of some or all of the following/l shampoo, scissors, and other cutting instruments and that such may result in injury if the animal being groomed moves suddenly. I will not hold the groomer or The Pet Doctor responsible for any injuries that occur during grooming. *

I understand that if my pet is matted, shaving may be required. Removing a heavily matted coat includes risks of nicks, cuts, or abrasions due to warts, moles, or skin folds trapped in mats Heavy matting can also trap moisture near the pet’s skin allowing mold, fungus, and bacteria to grow, causing skin irritations that exist prior to the grooming process. After-effects of mat removal procedures can include itchiness, skin redness, self-inflicted irritations or abrasions, and failure of the hair to re-grow. Shaved pets are also prone to sunburn and should have sunscreen applied daily or should be kept out of the sun until the hair grows sufficiently to protect the skin. In some cases, pets may also exhibit brief behavioral changes. As the owner, I am responsible for the condition of my pet’s coat and will not hold The Pet Doctor, groomer, or other staff, responsible in the event of adverse effects of mat removal. *

All grooming prices include a bath, nail trim, ear cleaning, anal glands expressed externally (dogs only). Prices may vary depending on coat condition and type of haircut (if requested). I understand that only the groomer can provide me with an accurate estimate of the cost for my pet’s grooming. *

I have understood and agree to the policies outlined in this document. I certify that my pet is current on Distemper and Rabies vaccinations. I have informed the staff of any history of previous aggressive behavior if applicable.
